New valproate safety measures apply from 31 January https://www.theabn.org/news/663716/ https://www.theabn.org/news/663716/ From 31 January 2024, new regulatory measures are in place to reduce the known harms from valproate, including the significant risk of serious harm to the baby if taken during pregnancy and the risk of impaired fertility in males.

Healthcare organisations were instructed by the Medicines and Healthcare products Regulatory Agency (MHRA) on 28 November to put in place a plan to prepare for these changes.

From January 2024, valproate must not be started in new patients (male or female) younger than 55 years, unless two specialists independently consider and document that there is no other effective or tolerated treatment, or there are compelling reasons that the reproductive risks do not apply. For the majority of patients, other effective treatment options are available.

All women who could become pregnant and girls who are currently taking valproate will be reviewed at their next annual specialist review, using a revised valproate Annual Risk Acknowledgement Form, which will include the need for a second opinion’s signature if the patient is to continue with valproate.

These are important regulatory changes recommended by the Commission on Human Medicines to increase scrutiny of valproate prescribing to reduce the harms from valproate.

New safety and educational materials have been introduced for patients and healthcare professionals to support the implementation of these measures. The MHRA has asked healthcare professionals to review the new measures and materials and integrate them into their clinical practice when referring patients and to aid in discussion with patients concerning treatment with valproate.

The MHRA urges patients to attend any offered appointments to discuss their treatment plan and to talk to a healthcare professional if they are concerned.

Clinicians should discuss the current warnings and upcoming measures relating to valproate with their patients and consider together how it affects the patient’s individual circumstances.

The ABN Services Committee aims to support the membership in driving forward excellent services in the UK. We work closely with the Neurological Alliance and the NHS England National Clinical Director for Neurology. The aim of regional representation is to ensure that information can be disseminated up and down systems. We meet bimonthly for 45 minutes. We are looking for an enthusiastic consultant neurologist for the following regions:

  • West Scotland 
  • Oxford 
  • Trent

Quality Committee

The ABN Quality Committee functions to advise ABN, through the Council, on all issues relating to quality improvement in neurology within the UK and Ireland. We are looking for Ordinary members with interest in developing quality assurance processes and improvement methodology within the ABN to support:

  • clinical practice guideline development
  • clinical and service audits in quality assurance, quality improvement and innovation
  • ABN processes such as maximising effectiveness in meeting ABN members’ needs revalidation

This includes treatment and support for people who are experiencing the first symptoms of a neurological condition, right through to people who have lived with such conditions for a long time. 

The pathways can be used to commission and improve local services to meet the needs of the local population and deliver efficiency savings across the system. They have been developed by consultant neurologists, with support from allied health professionals, patient groups and people affected by neurological conditions.

  1. “Entry” clinical fellowships up to 12 months, to provide clinicians transitional funding prior to starting a higher degree.
  2. Post-doctoral clinical fellowships up to 12 months, for clinicians undertaking research after their PhD.
  3. Non-clinical post-doctoral fellowships up to 36 months, for basic scientists embedded in clinical research teams.

Dedicated to Equality, Diversity, and Inclusion, I've implemented policies and collaborated on projects across Europe and Africa to enhance representation in European Studies. In 2023, I spearheaded the launch of UACES's new academic journal, Contemporary European Politics. New MHRA National Patient Safety Alert for Valproate November 2023 https://www.theabn.org/news/658780/ https://www.theabn.org/news/658780/ UK National Patient Safety Alert concerning valproate use in men and women. Whilst there is no immediate need for clinical action, it is important that you familiarise yourself with this new information; to help with this, we have published a guideline, explaining the new recommendations. They will be implemented in two phases; the first phase (concerning all new prescribing of valproate to men and women under 55, and all current prescribing of valproate to women under 55) will go live on 31.1.24.  Phase 2 (concerning all men under 55 currently prescribed valproate) will be implemented probably later in 2024, at present there is no fixed date.

The posts will give the fellow 12 months' accreditation towards their CCT in neurology. This annual scheme commenced in 2013 and parallels the very successful ANZAN Fellowships to the UK that have been running for more than 20 years.

The recipient Australasian cities vary and will be confirmed when applications open each year.

New optimal pathways of care available for services for people affected by neurological conditions https://www.theabn.org/news/644058/ https://www.theabn.org/news/644058/ New optimal clinical pathways of care have been developed across a range of neurological conditions, including movement disorders, headache and facial pain, traumatic brain injury and functional neurological disorders. 

The optimal clinical pathways, developed by clinicians (including neurologists) patient groups and people affected by neurological conditions, and endorsed by the Association of British Neurologists (ABN), set out what good treatment, care and support looks and feels like.  

Pathways intended to be inspirational
The pathways of care are intended to be aspirational, recognising that implementation will vary across the country. The pathways were primarily developed in England, although the elements of care identified apply across the UK, even if the commissioning and provider levers differ.
 
The suite of pathways have been developed by the neurological community, with the support of NHS England and the National Neurosciences Advisory Group (NNAG). NNAG was founded in 2016, and brought together all the different people and organisations that need to work collaboratively to achieve improvement in services for people with neurological conditions. NNAG wound down at the end of 2022 in light of changes to the commissioning and clinical leadership landscape in England.

The development of this pathway was overseen by NNAG, with input from professional bodies and patient organisations. A 6 week public consultation was held to gather input, views and experience from people affected by neurological conditions and wider stakeholders.

What next
The pathways can be used to improve local services to meet the needs of the population and deliver efficiency savings across the system. We urge anyone with an interest in improving neurology services to use the pathway and help improve care.

A 6 week public consultation was held to gather input, views and experience from people affected by neurological conditions and wider stakeholders.

The role has three main aspects:

  1. Oversight and support for quality assurance, improvement and innovation initiatives by ABN members

  2. Leadership and development of ABN initiatives in standards, safety, guidelines, audit, outcomes, work practice, revalidation, wellbeing and production of QI/QA resources

  3. Being part of the ABN Executive Committee’ – a subset of Council that provides active leadership and oversight to the ABN’s work in between Council meetings.

Although this role is unpaid, any related travel and subsistence expenses incurred on official duties will be reimbursed according to the ABN policy in effect at the time.
